Liability Waiver, Photo Consent & Code of Conduct

1. Acknowledgement of Risk

Basketball training involves physical activity and inherent risks including but not limited to: sprains, strains, fractures, concussions, and contact-related injuries. I acknowledge these risks on behalf of myself and my child.

2. Release of Liability

I release Perfection Basketball, its coaches, staff, and venue partners from any claim arising from participation, except in cases of gross negligence. I am responsible for ensuring my player is physically fit to participate.

3. Emergency Medical Authorization

If my child is injured and I cannot be reached, I authorize coaching staff to seek emergency medical care, including transport to a hospital. I am responsible for any associated costs.

4. Photo & Video Consent

From time to time, photos and short video clips may be taken during sessions for social media, marketing, and player feedback. You can opt out below — we still take you on regardless.

5. Code of Conduct

Players and parents agree to respectful conduct toward coaches, teammates, and venue staff. Disruptive behavior may result in removal without refund. Cancellations require 24 hours notice to avoid loss of session.

6. Payment Terms

Packages and season passes are non-refundable but may be transferred to another player with our approval. Drop-in sessions are non-refundable once payment is confirmed.
